1. Self Schedule an Appointment Online, available appointments can be found on the website 24/7.
2. Prepare for your appointment. All intake forms will be emailed to you once your appointment is confirmed. Please take time to complete these prior to your visit so we can devote all of our time to your concerns.
3. Telemedicine Visit you will receive an Email that contains your intake forms and a link for the Telemedicine visit. Just click on the link on your appointment day and time. Please have a blood pressure taken within the last 7 days to report at each visit.
