1. Schedule: Self Schedule an Appointment Online, available appointments can be found on the website 24/7.
2. Prepare: All intake forms will be emailed to you once your appointment is confirmed. Please take time to complete these prior to your visit, allowing us to devote all of our time to your concerns. Please have a blood pressure taken within the last 7 days to report at each visit.
3. Your Visit: you will receive an Email that contains your intake forms, and a link for the Telemedicine visit. Click on that link on your appointment day and time.
