Initial Intake visit - $275
Follow up visit -$150
Short Problem Visit- $50
INSURANCE CLAIMS
Vitalize for Women does not bill to insurance companies. Patients may submit a claim to
their insurance company for possible reimbursement. It is the responsibility of each
patient to contact their insurance for more information regarding reimbursement. A
super bill can be provided after each visit to the patient at the patient request, to be
submitted by the patient with insurance claim.
Labs can be billed through your insurance at the specific location of service.
PAYMENT
The initial intake packet contains a credit card consent for a credit card to be kept on
file. Your credit card will be billed in full after each office visit.
$25 CANCELLATION FEE for appointments cancelled or rescheduled less than 48
hours in advance.
