Cost of Services:

Initial Consultation visit - $250

Comprehensive Hormone panel - $150

Comprehensive Follow up visit -$195 includes lab review/discussion, evaluation of new treatment plan

Standard Follow Up Visit (refills or problem visit)- $95

INSURANCE CLAIMS

At the current time, Vitalize for Women does not bill to insurance companies. Patients may submit a claim to their insurance company for possible reimbursement. Patients will need to contact their insurance for more information regarding reimbursement. A super bill can be provided to the patient at the patient request, to be submitted with their insurance claim.

PAYMENT

The initial intake packet contains a credit card consent for a credit card to be kept on

file. Your credit card will be billed in full after each office visit.

There is a $25 CANCELLATION FEE for appointments cancelled or rescheduled less than 48

hours in advance.
